Maternity Services Feedback form

Please note, that the purpose of the Maternity Services feedback form is quite different to the one of submitting a formal complaint. The Maternity Services Feedback form is more like a survey, which our Maternity Services team continually checks to evaluate and to improve our Maternity services. So if you have any serious Maternity concerns or issues that need to be addressed and responded to by our Quality & Safety Patient Manager, then this form should not be used. In that case please submit a formal complaint, and then if you think, you would also provide feedback on our Maternity Services Feedback form - please do so.
